All Age Disability Team 0 to 25

All Age Disability Team 0 to 25

We are a specialist disability service supporting children, young people and their families in Southwark. We support individuals to live safe, independent and active lives as part of their local community. If eligible, following an assessment, we can provide support specific to individual needs. The services provided may include activities as set out in the Local offer, short breaks, care arrangements, or a personal budget.

When we receive information from yourself or other agencies, such as your child’s school or GP, an AAD practitioner will contact you to discuss your child’s needs,  and your needs as a carer.  We will seek your permission to obtain information from other professionals involved, such as your child’s GP, teacher, physiotherapist, if appropriate. When agreed we will arrange to meet with you to assess needs. We will discuss what services are available to support your child to achieve good outcomes. If your child is not eligible we will provide information on other services that can help.

After the assessment, if eligible, we will agree with you a support plan for your child.

As part of our statutory responsibilities, we will always work with you to ensure your child’s welfare is safeguarded and their needs are being met.

Who is the service for?

Children, young people and their carers who live in Southwark where disability has a severe impact, up to the age of 25.

Criteria for access to service

This is a statutory service provided by the Local Authority. The criteria for the AAD service in Southwark is that the child has a disability which is severe and profound, long lasting or permanent, or has a life limiting or life threatening condition.

A ‘severe and profound’ disability is where a child or young person requires significant additional support from another person, or equipment to carry out their basic functions such as personal or night time care, eating and participating in activities. As an adult it is likely that they will continue to require this.

Accessibility of this service

The AAD service is based at 132 Queens Road between 9am and 5pm, where our duty team is located. We do however prefer to visit families in their home as this is where children are more relaxed and therefore easier for families.
