Learning disabilities

Support provided by the Community Learning Disabilities Team

If you have a learning disability and live in Southwark, you may be able to get support from the Community Learning Disabilities Team to help you with the things that are important to you and have more control over your life. If you have eligible care and support needs, the team can provide the following:

Learning disabilities team
  • advice and information about what is available in your local community
  • support at home, for example learning to cook or help with your personal care
  • support to make sure you have appropriate housing
  • support to find a job or learning opportunities
  • support to access activities you are interested in
  • help with keeping safe at home and in the community
  • help to move from children's services to adult's services
  • shared lives scheme
  • support to help you stay healthy 

Specialised health support

You (and your carer/s) can also get help with specialised health support to help you to live active and fulfilling lives, including:

  • Audiology - hearing assessments
  • Community Nursing - health and wellbeing
  • Occupational Therapy - support to help with everyday jobs at home
  • Physiotherapy - help with body posture and movement
  • Speech and Language Therapy - help with communicating and eating and drinking

How to get support from the Community Learning Disabilities Team

To receive support from the team you must:

  • have a learning disability
  • meet the Care Act eligibility criteria

A social worker will meet with you to assess your needs. If you need help with this, independent support  can be provided.
