Health and social care privacy notices

How our health and social care services use information about you and how we protect your privacy.

Privacy in the London Care Record

The London Care Record (LCR) is a secure system. It shares your health and care data among healthcare and social care professionals. It helps us quickly and securely find the information we need to make the best decisions about your care.

The LCR contains your:

  • name, date of birth, gender, address, and contact information
  • NHS number, which helps identify you
  • GP details, appointments and visits to hospitals
  • current medical conditions, medications and medical history
  • results of tests and scans, as well as records of any hospital referrals, admissions, discharges, and clinics you have attended
  • documents such as health and social care plans, clinic letters and discharge summaries

Only health and social care professionals involved in your care can access your LCR. They must have a lawful reason to do so. This keeps your information confidential. It's shared only with those who need it to help you.

Visit South East London Integrated Care Systems to find out about the LCR.

Complete an individual rights form if you do not wish for your records to be shared through the LCR.

If you have questions or want to discuss your choices before filling out the form, call 020 3192 6011. Leave your name and number for a callback.
