Disability support
If your child or adolescent has a severe and permanent disability we can support them and you.
Our team is made-up of social workers, communication specialists and support staff.
We carry out assessments, plan services and provide social care support.
We work with other professionals to build a team around the needs of a child.
We also provide advice to parents and can refer them to other services as appropriate.

Professionals working with children
You should decide if a case meets the criteria for disability registration.
If it does not, we can provide advice on other services that may be able to help.
If it does, complete the inter-agency referral form. A social worker will be assigned to carry out an assessment.
It usually takes 35 days to complete and involves seeing the child at home and at school.
They will assess:

environmental factors that may be influencing a child's upbringing
