Intermediate Care Southwark

What is Intermediate Care Southwark?

The Intermediate Care Southwark service is a joint health and social care service. It can help you to remain independent, safe and well at home. It can also prevent the need for you to go to hospital, accident and emergency or into long term care.

The Intermediate Care Southwark service has a range of multi-disciplinary team members including:

physiotherapists
social workers
occupational therapists
nurses
pharmacists
rehabilitation support workers

The service can provide access to and support from a Consultant Geriatrician.
