Who we are
The older people and physical disabilities (OPPD) team supports:

adults aged 65 and over
-
adults aged 18 and over with dementia
-
adults aged 18 and over with a physical disability (18+)
-
their carers
You can make a referral to OPPD for yourself or on behalf of someone else.
We'll then work with you to decide on the best way to provide support.
This may include:

carer support at home
-
equipment or adaptations to your home to help you safely manage your daily needs

support and advice for people providing a caring role to a friend or family member
-
linking you to other community support services
-
supporting you to move into alternative care settings if you can no longer remain safe at home
If you contact us for carer support we'll arrange for a social care practitioner to meet with you to assess your needs.
To be eligible for carer support at home or in a care home you must meet the Care Act eligibility criteria.
