How to request a carer's assessment
If both you and the person you care for are over 18 you can be referred for a carer's assessment.
They can also offer advice and guidance to help you while you wait for your assessment.
This could include helping you find support groups and services near to where you live.
Everything you tell them will be recorded in your referral notes. So you will not have to share the same information twice.
Alternatively, you can refer yourself directly.
Who to contact
Get social care help has contact details for all our teams who carry out carer's assessments.
The team you contact depends on your needs. Teams work from Monday to Friday between 9am 5pm.
