Get an occupational therapy assessment

If your disability is more complex you may benefit from occupational therapy.

During your assessment the occupational therapist worker asks questions to explore what is working well at home. 

We'll identify your priorities, any changes you want to make and opportunities for improvement.

We may:

  • share advice and information about other services like housing
  • share strategies to manage everyday tasks like washing
  • recommend equipment that could help like equipment to help you get in and out of the bath or on and off the toilet
  • provide small (minor) or large (major) adaptations to your home, such as installing ramps or a stairlift

You can request an occupational therapy assessment by either:

Referrals

If you’re a professional making a referral on behalf of a service user complete the referral form for organisations.

If you need a major adaptation

Whoever owns your property is responsible for providing major adaptations.

The council is committed to making the Disabled Facilities Grant available to eligible:

  • owner-occupiers
  • tenants and landlords

This is so disabled residents can remain safe and independent in their own homes. 

The maximum funding available is £30,000.

The grants depend on your financial situation (means-tested). You may have to make a contribution towards the cost of the works.

You must have an occupational therapy assessment before major adaptations can be organised through the council.
