If you've had a fall, or are afraid of falling, ask your GP or another health professional to refer you to the falls prevention service.

Rehabilitation for heart problems
If you’ve had a heart or circulation problem and been treated in hospital, for example, a heart attack or heart surgery, you’ll be invited to a cardiac rehabilitation service. This includes exercise, education and relaxation, led by the hospital rehabilitation team. The service aims to help you:
- manage your condition
- regain your quality of life
- increase your physical activity and ability to exercise

Rehabilitation for lung problems
Pulmonary (lung) rehabilitation will help you if you're living with long-term (chronic) lung disease, such as:
- chronic obstructive pulmonary disease (COPD)
- asthma
- bronchiectasis
Pulmonary rehabilitation may also help if you've had COVID-19 and are feeling breathless, if your chest feels tight or feel tired a lot or all of the time.
Pulmonary rehabilitation is a course of education and physical activity that helps you learn more about your condition. It will improve your fitness and strength, and help you to feel less breathless. By understanding your condition and knowing how to cope with it, you can feel back in control again.
